You may know that there are two methods to changing your name, “Legally,” The first is the aged old common law recognized name changes when done for legitimate reasons (i.e. without fraudulent intent). The process was complete after an individual had begun using his/her new name on a consistent basis. Of course one would have to be prepared for times when proof of identification is required, some may accept a recorded document detailing the new name, but others may require a formal document, which leads to the second method. In such cases, the State of Arizona requires a formal document which provides an accepted legal name change that must be obtained through a court order issued by a judicially recognized body in the state of Arizona.
The proper jurisdiction for the legislative legal process allowing a person to change his/her name can be found at Arizona Revised Statutes §12-602 Notice of application; effect of change on rights and obligations (Link) (Click here for a complete text of the statute).
